Cervicogenic Headache · Recovery guide

Understand the pain. Plan your next step.

Cervicogenic headache is a headache attributed to a neck disorder when assessment supports a causal link. Neck pain alone does not establish the diagnosis: migraine and other headaches can also involve the neck. Medical assessment helps distinguish the cause before appropriate physiotherapy is considered.

Cervicogenic Headache: What should I know?

Cervicogenic headache is a headache attributed to a neck disorder when assessment supports a causal link. Neck pain alone does not establish the diagnosis: migraine and other headaches can also involve the neck. Medical assessment helps distinguish the cause before appropriate physiotherapy is considered.

Know when to seek help

Some symptoms need medical care first.

Seek emergency care immediately for:

  • A headache that starts suddenly and is extremely severe, or occurs with seizure, weakness, confusion, loss of vision or difficulty speaking or walking.
  • Headache with high fever and a stiff neck, or after a significant head injury with concerning symptoms.

Ask for prompt medical assessment · 1

New or worsening headache, repeated vomiting, visual symptoms or jaw pain when chewing needs prompt medical advice.

Ask for prompt medical assessment · 2

A headache pattern that keeps returning or changes from your usual pattern needs a medical appointment.

Understand the condition.

A diagnosis needs a supported link

Having neck pain or an abnormal neck scan is not sufficient on its own to prove that the neck causes your headache.

Migraine can include neck pain

Different headache types can overlap. Appropriate medical diagnosis avoids treating every headache as a posture problem.

Rehabilitation has a specific role

Where a neck contribution is established, selected mobility, strength and activity work may help. Physiotherapy does not replace headache medication review or urgent medical care.

Assessment & shared decisions

Your symptoms. Your daily life. Your plan.

We ask about headache onset, frequency, associated symptoms, neck symptoms and medical assessment. Bring a symptom diary and relevant reports if available.

After checking warning signs, we assess appropriate neck movement and function. We may recommend medical review to clarify the headache type before agreeing a rehabilitation plan.

  • Private assessment with a female physiotherapist
  • Discuss your goals and relevant health history
  • Understand when medical review is needed

What might you notice?

Symptoms can overlap with other conditions. An assessment helps clarify what needs attention.

01

Headache with neck symptoms

A headache may occur alongside neck pain or limited neck movement, but this pattern is not unique to cervicogenic headache.

02

A relationship to neck movement

Some symptoms relate to neck movement or a neck disorder. A clinician must assess whether that link is meaningful.

03

A changing headache pattern

A new, unusual or worsening headache needs medical review; do not assume the neck is responsible.

A pathway shaped around you.

These are flexible goals, not fixed weeks or a promise of recovery.

  1. 01

    Understand & assess

Discuss your symptoms and goals. Check warning signs and agree an appropriate starting point.

  2. 02

    Move & build

Adapt everyday activity and practise selected movement or strength work. Adjust the plan to your response.

  3. 03

    Review & continue

Review useful changes in daily function, refine the home plan and seek further assessment when needed.

Home exercise & everyday activity.

These are planning ideas, not a universal exercise prescription. Your clinician selects the movements, range and dose after assessment. Stop and seek advice for new or increasing numbness, weakness, severe pain or the warning signs above.

Keep a headache diary

Find a manageable starting point with your clinician.

  • Record timing, duration, associated symptoms and activities. Share changes with your clinician; a diary supports assessment, not self-diagnosis.
  • Agree which movement or task is appropriate for you.
  • Track how symptoms and daily function respond; ask for review if they worsen.

Movement, frequency and dose are individual.

Vary your working position

Find a manageable starting point with your clinician.

  • Use comfortable breaks from prolonged desk or phone tasks rather than holding a rigid posture.
  • Agree which movement or task is appropriate for you.
  • Track how symptoms and daily function respond; ask for review if they worsen.

Movement, frequency and dose are individual.

Follow selected neck movement

Find a manageable starting point with your clinician.

  • Only use movements chosen after assessment. Avoid forceful self-manipulation or exercises that bring on a different or severe headache.
  • Agree which movement or task is appropriate for you.
  • Track how symptoms and daily function respond; ask for review if they worsen.

Movement, frequency and dose are individual.

Your questions, answered.

Does neck pain prove my headache comes from my neck?

No. Migraine and other headaches can include neck pain. A cervicogenic diagnosis needs evidence supporting a causal relationship.

Will a neck scan diagnose it?

Imaging alone is insufficient. Neck changes can occur without headache, so a clinician considers the history, examination and supported connection.

Can physiotherapy treat every headache?

No. It may have a role for an assessed neck contribution, but other headache causes require appropriate medical management.

How long will improvement take?

There is no single timetable. Your symptoms, health, daily demands and response guide progress. Reviews focus on useful changes in function and whether the plan needs adjustment.

What should I bring to an assessment?

Bring relevant reports, a medication list and details of previous care if available. Tell us which daily tasks are difficult and any new or worsening symptoms.
